How it works
Four steps, a few minutes.

- Enter the treatment plan amount in the field provided.
- Show the page to your patient or print them a copy.
Required for amounts of $3,000 and up.
- In your portal, click National Bank Form.
- Click Download to print the document.
- Print a copy and have your patient sign it.
- Upload the signed copy to the request.
- Go to your portal, here.
- Simply fill in the required fields.
- Upload the signed National Bank form (if $3,000 or more).
- Click Submit.
- Go to your portal, here.
- Find the contact person's details in the For more information section.
- Check your patient's key information in the Patient section.
- Access contract details and the disbursement date in the Financing contract section.
